Yes! We are proud to support the 2022 Cook County North Suburban Annual Spring Gala. We authorize NAMI to include our name and/or logo on all the materials printed for the event consistent with our sponsorship selection.

For guaranteed inclusion of name/logo on printed materials, return form by April 1, 2022. Your logo should be sent in the following file formats: (EPS, PS, PDF) must be received within 5 weeks of the Gala, otherwise you will be listed by name only at the event.
